Migration generator - fix ordering of index changes vs table/column renames

On at least MySQL, dropping a column also drops indexes attached to that field (who knew?). We either need the hardened remove_index that ignores missing indexes or we need to get the order right (drop index, then field).

(from [560ad23eaf5d5b20f88805b110d6dafb13df4850]) [#566] in mysql, removing an index on a column that doesn't exist fails. There are better fixes possible, but this is the easiest.

OK, it's fixed, but I've left the bug open in case we want to make a better fix.
