Ask user before dropping indexes

(continuation of #555)
The ignore_index option added in #555 may not be ideal for all users - it would be good to have an interactive verification like for dropping tables and fields.
The gotcha is that the current code detects indexes on a renamed table as a drop/add pair, and we'd need to filter those out to avoid nagging the user (who wants to explicitly rename autonamed indexes?).
