Card should always show a heading+link if there is a show action

When a card is displayed on the Index page, the name attribute is used to link to the card's Show page. But no link appears if the model lacks a name attribute.
The Show page uses a generic title such as Document 1 for such models - it would be good if the index pages had the same behaviour.
