create foreign key constraints automatically

The correct way to solve this would be to add something like create_foreign_key_constraint to the connection adapter. That way it can be implemented differently for each database.
We could then extend the migration generator to do create_foreign_key_constraint along with creating the actual foreign key column for each belongs_to
